Rip-off dishonest billing on closed account

I cancel my account back in early 2000. And since that time I do not remember to open another one, especially since you had breach in security that exposed customer names, addresses and portions of credit card numbers.
Letter also says about me not fulfilling my commitments, which I did.

How is it possible to get a bill? Probably because your computer have some mistake, and just opened all closed accounts again.

Is it identity thief?

Even your web site http://www.columbiahouse.com says? Sorry. We did not recognize your Account Number. Please try again.?
