Sending negative remarks to the credit agency for a card that i don't recall having

They have a negative remarks on my credit report about having a credit card with them and I keep telling them it wasn't me but they put it on my credit report and I have been trying to get it straight for a long time. It shows that it started off at 400.00 opened in 1998. And now its up to 3049. But said it was a charge off on 9 of 1374. And this is messing me up terribly and I don't know what to do because I have tried a repair agency and that didn't work to well.
