2nd time t mobile has accepted false credentials opening an account for someone else using my name

I was a victim of identity fraud. Three months ago i recieved a bill for 499.00 dollars from t mobile. This shocked me, seeing how i was in applying for a phone two months prior, and was denied, due to my credit. Well im not sure how it happend, i can be me and get no phone or i can pretend to be me and get two. (go figure). The $499 bill was from someone else using my name, soc. Sec. Number and address.

After fighting with t mobile for a few weeks they admitted they knew i was not responsible. They found out the account was fraudulent and shut it off before i even spoke to them initially. They fixed this error at no cost to me.

Now, once again, the same thing has happened. I have received another bill with a different phone number but with my name and address. Apparently someone has opened another phone account in my name. When this happened the first time i let them know that i would never, ever have an account with them. I said that if someone else attempted to open an account in my name they should send up a flag immediately. How is it i have received another bill for a new account?
