Identy theft and fraud

I had run my credit report and i found out that i had an att&t bill send to collection for 3 lines that i had never requested as a matter fact i do not even have an att&t services always was with spring and then went to veriozin (page plus).
I had call the company and requested proof of this order or any documentation such original signature in a contract photo copy of my id or social security but they were not able to send me any of this things instead they send me a bill statement pass due from att&t which again this is what i am disputing is not my in no were there is my information except my name.
I will like to report this people for doing fraud unethical buisness practice and saling my information to a collection department inlegally prior to any type of verification that this account is my. As it is i have not been living in orlando i have been living in nevada.
I need your assistance and see what i can do for this people to correct there wrong doing and to fix my credit report imediatly. It is creating a lot of damages financial headaches. This is already taking one year and more to solve this matter
