Unauthorized withdrawal of funds from bank acct

On 10/9 in ckg my bank acct, found a withdrawal of $89. Ck #18029279. I called the bank for info on whomever withdrew the funds and was given the above phone #. Called the # and explained i do not know who this co is and had not authorized any withdrawal. How they got my acct info, assuming was by cookies. I just have no idea why, who, just want my money returned. I was told i needed to go to their web page and fill out an cancellation form and fax a copy of bank statement showing the amount withdrawn. I completed this all on 10/9, the day i saw the withdrawal completed on 10/9. I was told completion had to be done within 14 days that withdrawal was done. I did this same day. I emailed, called and emailed, but still have not received any response, i am a senior citizen and this money is a precious commodity to us and certainly want the funds returned now.
