Losing my personal health information

Faxes I ship for extra information on costs I've currently employed my FSA card are received. Faxes I ship for the money I paid of wallet plus they are designed to pay me for amazingly disappear. How reckless does a business need to be to get rid of my protected health info? Include that towards the proven fact that I've to email their customer support division 2-3 times in per week before I actually obtain a reaction. Subsequently their customer support division lies in my experience stating they've not obtained a fax from my number in eight weeks, but the fax I delivered them last month, in the same quantity, for one more info demand was obtained without problem. I'd prefer to understand what they did with my protected health info and where it's today.
