Glass bakeware shattered with enough force to smash my oven doo

My husband pulled dinner out of the oven Saturday night, and the baking dish broke with enough force to not only send glass flying 10 feet, but also with enough downward force to smash the interior oven door.

The baking dish was by Anchor-Hocking, was less than 6 months old, and WAS used as directed. The oven was set to 375, it was preheated, the dish didn't have any cracks or chips, and went from room temperature into the oven.

Dinner was ruined, the dish was ruined, and our oven is ruined, as you can see in the photos attached.

As far as injuries go, my kids and pets are fine, my husband suffered some minor burns, and I suffered glass slivers in my feet, which I am going to have removed at the doctor tomorrow, as well as a few minor scratches.

It was a 9x13 clear glass baking dish, purchased as part of a set that came with a 9x9 (or 8x8?) pan, a loaf pan, and two small round dishes with plastic covers.
