Teller Refused Check Because Account Name Differed From Check

I have had an account with bank of America for about a year or less and have deposit my check in the bank with no problem, but on 7/6 I had a problem putting my check in the bank because my account has the name [removed] and my check is [removed], according to the teller the computer had both names but the account was setup as smith only and she could not take the check.
The ATM/debit card was in [removed], the customer service rep said the person who issued my card could put any name on it
I asked her to check the computer to see the name on the checks I had put in the bank before, she made no effort to assist me, and with a smile on her face she continued to not help me. I was very upset so I told her I wanted to close my account she never tried to resolve the matter or offer me an alternative. She was very happy to help me close my account.
