Card closed after 0% offers with four false reasons

My wife had a Citi Mastercard with a $2,500 limit that allowed us 0% interest for 3 months on purchases over $200 and 0% interest on purchases over $350. We paid every bill we had over those amounts with our card.
After 4-5 months of this, my wife was sent a letter from Citi telling her that they had reviewed her file and "have found it necessary to close your Citi Mastercard account." They list 4 reasons that are completely false, even saying she has a "number of accounts with delinquency," which is 100% false. My wife has 0 delinquent accounts.
We have had this card for years.
