REFUND Receipt

My name is rosalinda avalos-robinson, im am writing this because i need directv to send my card holder a receipt of a refund that was placed on my card. They can not unfreeze my card without proof that the refund was meant for me. I have already spoken to a representative various of times and have already told me they do not send out receipts and directed me to write this in order to have a receipt of the refund sent to my card holder. The fax number is 626-739-2057 and the ticket number is 668155. I urgently need this sent to them so i can have access to my card.
