My name was used to open an account and ruin my credit

Our name was utilized by a co worker to determine a merchant account with Columbia Home DVD Club, plus they declined to confirm any private information about me so that they might correct the issue. They stated they'd shut the consideration (IN MY OWN NAME) and my credit could be destroyed. Please help!!!
