Mr. Niberts account and cancelation charges

Who had an account with you {Direct TV } account #: 70589486, Phone #: 702-302-1687, Billing Address, 261 Sunvalley Dr Apt # D13 Mesquite NV, 89027

On or around the end of April Mr Nibert signed up with the intentions of honoring his contract to not cancel service for a set period of time. Approximately two weeks later Mr. Nibert suffered two strokes and was hospitalized at the Veterans Hospital in Las Vegas NV. On or about four weeks later Mr. Nibert was transported back to Mesquite NV by ambulance and admitted to a Rehabilitation Center (Highland Manor). Due to Chronic Alcoholism Mr. Nibert had to relearn your basic skills. Ie, Speaking, walking, eating.

On July 26 Mr Nibert was released from The Highland Manor under the care and supervision of his Daughter at her home. Mr Nibert did not return to his home at 261 SunValley DR apt D13. His Daughter was unaware of Mr. Niberts contract with Direct TV and when she asked Mr Nibert if he needed her to cancel services with Direct TV Mr. Nibert stated he thought he already canceled service but was not certain. Mr. Nibert is on a fixed income and was unable to pay this months living expenses due to the fact that Direct TV withdrew $550.00 and then again on the same day withdrew $97.08. This was an unforeseeable and unavoidable circumstance not a Deliberate Breach of Contract.

Once Mr. Nibert is fully capable of careing for himself he would like to continue doing business with Direct TV In light of Mr. Niberts current mental capacity I am respectfully requesting that Direct TV refund Mr Nibert the $550.00 and the $97.08 that was withdrawn from Mr. Niberts Bank Of America Account on 11/5 and only charge him for the actual service.

Mr. Niberts Daughter is in possession of Two Cable Boxes and is requesting the information needed to send them back to Direct TV. Sincerely A Rep for Mr. Nibert
