Gift card not received

We switched to twc from AT&T u verse in sept and were promised a $50 Visa card. We did what we were asked to do. We never received the card we called this am and after holding for 20 min someone answered and "transferred" our call which was promptly dropped. I did a live chat with twc and was given a phone number who told me they had no record and it had to be in oct27. Twc then said I could go to a web address and register again. The site asks for copy of last AT&T bill which we don't have. TWC says switch and we will give you $50. Not exactly
