Customer Service, only if you're buying something. A study in frustration

There are several issues I need to report.
1. I requested information regarding Universal Lifeline service several weeks ago. I was told I would receive the forms by mail in a few days. While I wait (and wait), I'm being charged the regular rate which I cannot afford.
I've had Universal service in the recent past. My service was discontinued because I forgot to sign a form properly. Why not just send me the form which I could sign and return?
2. Someone has tapped into my Go Phone Wireless account. My balance has dropped anywhere from $10 to $20 overnight. I cannot afford this.
I've requested a hard copy of my wireless calls to find the culprit. Staff at the local AT&T store were less than helpful. I was given a card with a Web address. At this address I was informed that my request could not be done online and I was referred to AT&T Customer Service. Here, there is no tab to request copies of bills.
3. Someone has hacked into two of m y e-mail accounts and inundated them with SPAM. After speaking with someone at Customer Service about directions to close these old accounts, I was given a phone number to call. That number has been disconnected. Another dead end of useless advice.
