Falsely reported to Verizon that I had used their services

On this month's Verizon bill, there was a $13.51 charge from an OAN Services for two calls to Directory Assistance to 101-515-800. Neither I nor my wife has made such calls. I have deducted the charge from this month's payment to Verizon, but forsee a dispute. Verizon must be aware that these charges are improper, yet they allow them.
Would appreciate any advice on how to best defeat this scam, and prevent recurrence.
