Promised $150 gift card rebate never received after signing up

I signed up for Dish on 1-1-15. There was a televised ad as well as a Dish website ad stating that after 6-8 weeks of service I would receive a $150.00 Visa gift card (code Get150). I have complained numerous times and was twice told it was going to be resubmitted. When I complained again, I was told I do not qualify. It is now 7-1-15 and nothing has come of it.

I was verbally offered free HBO for 3 months, which I already had in the Everything package, and I was offered $5.00 off my bill for 6 months. This is a joke! Also, I have been charged for movies I never ordered. Something needs to be done about these companies!
