Rebate Repeatedly Denied for Wrong Form and Missing Number

Refused to issue $50 rebate on my phone citing I submitted the wrong form. When I resubmitted the "right" form, 2 months later I got another letter denying the rebate this time citing that my number did not exist in their database. They are finding as many ways as possible to deny the rebate.
