Identity Theft

On August 10, I was contacted by Jaclyn Kelly of Applied Merchant Systems, 312-789-4937 ext. 6775, regarding a credit application that I had allegedly filed with her company. This application included my personal information, including social security number, drivers license number, date of birth, etc. The business name being used for this application is Best Stationers, www.beststationers.com, 1307 Northdale Blvd., Tampa, FL, 813-345-4818. The IP Address for the website listed is 64.15.133.193 and is traced to Pakistan. I am not affiliated with this business and did not submit this credit application.

It is evident that someone has gained my personal information and is attempting to use it. I have contacted all three major credit companies and placed a freeze on my credit and have cancelled all credit cards. The perpetrators also provided on the credit application active bank information. The account provided is with Stearns Bank, St. Cloud, MN, routing number 091910455, and account number 124000010. I contacted the bank and this is an active account. However, the account is not in my name so they could not provide me with any further information. I assume that the holder of this account is linked in some manner my identity theft
