Checking account fraud

This company debited $298 out of my account by electronic check for a medical benefits card. I have insurance through my employer and have never heard of these people when i called the 800 number given to me by my bank i was told i would receive a call form the billing dept. A guy from a unknow number called me and advised i would be issued a refund but he never addressed any of my personal info with me other than my name so how did he know where to refund my money to? He put me on hold and the phone hung up i called back and got a answering machine and was told to leave a message. He coudlnt tell me when i signed up, or give me any information about this benefit card.
