Dazzle Dental Will not refund my overpayment

On February 18th, I took my son to Dazzle Dental in Anthem for some dental work. They claimed that they did not know whether my dental insurance would cover white fillings, so they would have to charge me $549.60 for his work. They assured me that they would be able to refund me the difference once my insurance paid them. My insurance covered $340.20 of the bill. I went into the office and was told that I would need to fill out a form requesting a refund due to overpayment. This was on April 6th. It is now May 25th and I have yet to receive a refund.
