Lied to asuris and filed report that i had refused lantus insulin, demanded cash payment in advance

Contacted asuris for payment, on next order reported to asuris that i had refused the lantis.

Did not contact the doctor on new rx for the lantis, tried to bill for 90 day supply of lantis 195.00
the lack report to asuris insurance co. Did not meet the requirements for the plan and therefor no co-pay
refused to give price for the lantis so that it could be paid for.

Assumed that i had met the requirements of the plan and would not fill the lantis until the first friday of three weeks with out insulin for the third time in three months even though they were paid cash for each rx
