Unauthorized contract

I'm looking for our what sort of new contract might have been started in my own title on 203-733-1611 without my acceptance though it plainly says on my consideration the NO BODY but myself may approve any change that will need a fresh or prolonged agreement.

Obviously, a Verizon affiliate in another of their shops possibly didn't study consideration records, dismissed them, or falsified the info.

Strangely, the site for “Contact us by email” for verizon won't fill.
