Verizon False $20 Customer Loyalty Mail-in Rebate

I purchased a cellphone service from Verizon, in April of with the understanding that I would receive a $20 rebate if several conditions were met. I followed the instructions and sent [I made copies for myself] the following information to "$20 Customer Loyalty Mail-in rebates":
1. Filled in rebate form; with signature
2. Copy of dated sales receipt [4-6-07]
3. Copy of 12-digit UPC bar code form wireless phone box [shipping bar code]
4. Copy of wireless bill dated 150 days or more after the Activation Date on
Sales Receipt; date-due of bill was 10-30-07
5. Mailed in rebate form with all documents required
