Specifically,
By agreeing to and meeting the Terms & Conditions as specified by WIREFLY Mobile (an agent for Verizon), I have transmitted the particulars as specified in their $150 Customer Loyalty Rebate FORM as follows:
1. Filled out the applicable form provided by WIREFLY
2. Read, signed and dated this form
3.included the following information with said rebate form:
- Copy of the dated sales receipt inculded with shipment
- Copy of the 12 digit UPC barcode from the product packaging
- My wireless service bill for this account dated between 15-210 days after the activation date of 01/12
Since this was an ONLINE Bill, it showed the following information:
-Date of invoice (bill or statement date)
-My name Donald R, which matches the name on my sales receipt and rebate form
-My mobile phone number 626- (ROR REDACTED FOR SECURITY PURPOSES) at the time of submission
-a SNAP showing ALL previous balances PAID IN FULL
4. Submitted completed rebate form AND ALL documents noted above to the following address on 07/12 as requested in the Terms 7 Conditions:
