Policy cancelled over a questionnaire I never received

The company cancelled my policy over a questionnaire that I did not fill out and send back to them. I have never received one. I found out about this questionnaire only from the cancellation note.
I called my agent, Thomas Esposito. He promised to try to help me extend my insurance but never got back to me. He claims to be too busy to call me back, but his strong suggestion was to change insurance companies.
