Cancels my policy for the questionnaire that I did not fill and send back to them. The questionnaire was never sent to me

Company cancels my policy for the questionnaire that I did not fill and send back to them. I have never received one. I've found out about this questionnaire only from the cancellation note.
I called my agent Thomas Esposito. He promised to try to help me to extend my insurance but had never got back to me. He claims to be too busy to call me back, but his strong suggestion was to change Insurance Company.
