Insurer signed my name on exclusion form and refused to pay claim

I first discovered that I was labeled as excluded driver on my policy. The insurance persons at United Autombile insurance signed my name on exclusion letter as me being insurer now they have tried to lie and say that I was excluded reporting it to the depart of Insurances. Forgey and insurance fraud and they are refusing to pay for a claim. On May I was faxed a letter from them stating that they had refused to pay for claim and didn't inform me of this issue until now. They are a very bad insurance company who lied and won't pay for insurance claim which is resulting in lawsuit for the other party against me.
