Reported accident and requested a claims form on phone, to be faxed to me, in Januasry accident was in end of December followed instructions, faxed back to them. End of Febuary called for status, they denied receiving info or fax to claims department, they faxed wrong forms and gave wrong fax number to me, yet, it was my fault.

I requested the correct forms and phone number to be faxed to me again to start over.By this time, medical bills went into collections. All forms and info submitted by fax, a second time.By the way, fax number was same as the first time.

Made it very clear I was cancelling my plan in febuary to everyone I talked to, which I was passed over to at least 4 departments. Each one said, we will take care of it for you, if you are not happy with it.

It is now the 27th of March I called for the status on my claim again. They did another automatic withdrawal from my bank account as if I never cancelled it.

Called them again today, membership says, if I don't know the names of all the people I talked to, she can't do anything about the cancellation. Won't refund the payment. I am STUCK for $123.44 again.

The second time I faxed the claim forms and info was on March 1. It took them till the 15th, for the underwritters to get it. According to membership. They say, they have no idea when or if it will go through. For me to call in a couple of months and recheck on it with them.

Contacted my bank, payment to them was pending to them, there was nothing they could do except pay it. For me to call back tomorrow and put a stop payment on it for future payments. So, bank refusses to help as well. What can I do?
