I worked at this location August August. I have had issues with them as an employer from day one and it's still going on now (February). I was hired to start as a hostess and later transfer to serving or drive thru. My first day I was put on fountain (making shakes). I was left in this position until January or February only after developing carpal tunnel from working with ice cream that was noted to have been kept too cold.in November I developed a cyst on my wrist, and my arm wound sporadically go entirely numb from my wrist to my shoulder. It would turn a purple-blue and I could not grab small objects if I was not looking directly at them. I was also unable to use that arm to drive or hold a phone for more than 15 minutes. I was told I needed to suck it up.

I finally saw my personal doctor, who after some simple tests said I had carpal serious enough that I should file Worker's Comp. All of the managers told me that there was no option to file a Worker's Comp claim if it wasn't a particular incident (ex. A fall, a burn). Knowing that would be the only way to possibly be moved to a position where I wouldn't be using my hand as intensely I found out through the Florida Dept. Of Labor who to contact. I filed over the phone. I got to the approved Worker's Comp doctor (Solantic, SR 200, Ocala, FL). The doctor said there was definitely a problem with my arm but I would have to see a neurologist to confirm exactly what. The approved neurologist (which was in Leesburg, FL) said the tests were normal. I still get occasional flare ups.

I admittingly quit suddenly, dramatically, and without notice in August. A mix of several employees making it clear they thought I was faking my injury, working several 12 hour BREAKLESS shifts and being passively accused of stealing (it turned out to really be a girl who had a criminal record that even filed a false complaint about me) pushed me over the edge. I walked out a half hour before the end of my shift, never returned.

I got my W2 in the mail a few weeks ago. I noticed that my social security number was incorrect. I went into the store, polite and professional and asked the manager to fax in the form for the W2C, as the back of the W2 instructed. I was told it was not his responsibility and given the number to payroll. Apparently my number was incorrect last year as well and I have to redo my taxes for 2009 as well. I emailed payroll the information they needed for the correction 3 or 4 days ago and no response as of yet.
