Retirement Account Transfer Treated as Distribution, $15,000 Lost

I requested a transfer of my retirement account and they treated it as a distribution when I did the paperwork exactly as they indicated on 2 distinct telephone calls. When I complained, the supervisor said they are not responsible for providing bad instructions. I'm more than $15,000 out and they simply don't care.
