Forced Flood Insurance Threatened Despite Faxed Proof of Coverage

I've always paid our mortgage (principal and interest only) by the 1st of the month.

However, each August CitiMortgage writes to tell us that they did not receive proof of flood insurance from our insurance carrier. I pull my file, find my July flood insurance declarations page, see that a copy was sent to CitiMortgage, and fax a copy to CitiMortgage anyway, to the fax number provided on the letter.

Within weeks of sending in my proof of flood insurance, I get an uglier letter from Citi saying that they did not receive proof of flood insurance (the declarations page I faxed to them) and that they will be creating an escrow account and forcing a flood policy on me (at four times the current market price of a like policy), which will increase our mortgage payment significantly. I call the phone number on the letter, and the Citi employee tells me "nothing was received."
