Scam! Repeatedly placing expensive forced insurance on non-escrow accounts (falsely claiming no proof of insurance was provided)

I've always paid our mortgage (principal and interest only) by the 1st of the month.

However, each August CitiMortgage writes us to tell me that they did not receive proof of flood insurance from our insurance carrier. I pull my file, find my July flood ins declarations page, see that a copy was sent to CitiMorgage, but fax a copy to CitiMortgage anyway to the fax number provided on the letter.

In weeks after sending in my proof of flood insurance, I got an uglier letter from Citi saying that they did not receive proof of flood insurance (the declarations page I faxed to them), and they will be creating an escrow account and forcing a flood policy on me (at four times the current market price of a like policy), which will increase our mortgage payment significantly. I call the phone number on the letter and the Citi employee tells me "nothing was received".
