5 year plan - more years of service

I purchased a magicjack plus and a five year plan all at the same time.

It's now a year later and I'm getting a message saying my subscription is ending.
Where are the four other years I paid for?
I purchased this due to my income going way down to only 1,500/mo and antisipated in advance, I could not afford ATT so I went this route. But, what do I do now?
I have no phone, no ATT, soon no MJ yet I paid for 5 years and there is no one to contact except the live chat and you get two words typed and get disconnected.

Can anyone help me?
I have MS and it's imperative I am able to contact my doctor and call in my prescriptions along with my other medical needs needs.
