Won't Give Refund

I'm a 71 year old with multiple health care issues. I hired someone to help me on the vacation and the airports. He had an accident so was unable to go. They refuse to give me a refund for his fare. I had to hire someone else but they wouldn't allow me to change the name of the person attending me. I rebooked for the new person at an additional cost. Now we have seats in different places on the plane. I really can't afford to be paying two people for the same thing. Why won't they refund the money i paid for the first ticket?
