Charged me services i didn't approve

They've been getting me to get a long-distance providers that I've never authorized since August. It price $16.88 taxes= $22.71 per month. I discovered It Might a cost on my phone bill. I named my phone business they explained to contact them. I named in-May these were to deliver me a document they never did. I got billed again in June. Named them again they did deliver me mail to were I accepted this. I never accepted this since my protection issue was moms maiden name they said Patterson. That's not my moms name. However awaiting a credit on my telephone bill.
