We received a solicitation requesting us to fill out a "2012 Annual Minutes Record Form" with a Due Date of 30 November. The solicitations appear as if they are from a NY State Agency. The information we received seemed to be generated (although not directly implicated) from the Department of State or another State Agency and not a private firm with no State affiliation. They request payment of $125.00 to accompany the Report due back in. This is a total scam. Do NOT send any money to this firm or send the report back to this firm.
