Refused to give accident report copy, on 11/09/10, slipped and fell on soapy wet floor, landing on my back, employee saw it accur, the caution sign was 8feet away from soapy floor, at 1:30am on 11/12/10, ha

While shopping, and pushing a grocery cart, I suddenly had my feet go out from me, and landed on my back, I than could see I was in the middle of a soapy puddle of water. Than a worker asked me if I was ok! I told him I wasn't sure. Than Iasked him to get the manager, and make a report just in case. He was a real prick, he treated me like I was to blame for falling, he took a picture of the area after the boy wiped up the mess on the floor. He took my information and said the store would investigate the incident. He refused to make out a accident report for myself. Than on 11/12/10, at 1:30am, I called 911 and was rushed to the hazel hawkins hospitol, emergency room for severe pain in my lower back and abdomin area! Jeffrey, 831-637-0986
