Charged for service, after I cancel. I was charged for an adapter that I wish to return and a cancellation fee that wasn't told to me in the installation of services

My complaint/dispute

On march 3, i was given this cancellation reference no. 17857092, a schedule cancel of service on 5/7. I was told this service would be put on hold for two months (until 5/7), without any fees (services), unless i call and cancel the cancellation. But instead of honoring the oral agreement of two months, which was really given me one month, because in february, i had prepaided for march and than in april i was charged for may, i called on 4/9 in reference to the fee and was told that it was for the next month of service (may).

I explain my earlier agreement and i didn't have to pay, since i had called in and cancel. I told this to the person on the phone: i already have a cancellation reference no. She refuse to take the number but stated a notation was on record for service to began on 4/7/09 instead of ending 5/7/09. My payment would not be refunded. Then she proceeded to give me another cancellation no. 18660458, along with the charges of $96.28. ($49.99 for the adapter, $39.99 for the cancellation and $6.30 taxes) i told her i didn't need the adapter, i will return it. I was ignored again by customer service. So after our conversation, i proceed to asked for a ra number to return the adapter thru email and was sent the reply below but still haven't gotten a ra number to ship this adapter.
