I supposedly confirmed to have thier services for long distance while filling out a survey online on facebook - no where did i agree to pay for their services!

I noticed an extra charge on my ATT phone bill this month and ATT said that I must have given approval at some time to have this third party long distance service added at some time. ATT told me that to keep this from happening again I need to fill out paper work that allows ATT to block any slamming from outside companies.
When I called USBI, they told me I signed on to their service when I filled out an online survey (which was when I was on facebook), and I know for a fact that no where did it say that I would be charged for anything. I also know that I didn't sign up for long distance because I use a cell phone and not a land line for calling out.
I want people to know to not fill out ANY surveys and to ask their phone carrier to initiate blocking slamming regardless if they have had any problems yet or not!
