Unable to activate, pay disconnect when not activated, continued billing

I ordered Vonage in November after lengthy conversation with their Sales Rep. The lines were unable to connect or activate so they were returned. After many calls, and a confirmation that all accounts were cancelled in January, they charged me a disconnection fee and continued to bill my credit card thru March. They continue to bill me for no service.
