Esbi charged me$14.95 on my phone bill without my permission

This company charged me $14.95 without my permission. This charge was on my Embarq phone bill. I did call and cancel, in that process I learned they had opened an account under my company name on my personal phone. How can they do this? Why does Embarq let them?
