Gems stones being missing after one to two wears - Return service charges

I have been buying from Gemstv for years. The items I bought initially were of excellent quality regardless of price, however recently the stones are falling out at the first wear.
Grant - I sent the first back and it was replaced immediately.
Garnet neck wear just snapped, Do not know what to do.
Help please.
