Ripoff fraudulant use of internet by adding phone charges

While filling out a survey to win a computer this company got enough information to fraudulanty add a voice mail service charge to our phone bill which we had no knowledge of. We called our phone company and was put in contact with them. They said they would reimberse us the charges for three months that we did not pick up right away. The survey would not let us complete each page But, got enough personal information to start charging us for something we had not ordered nor had any knowledge of.
