$10 Gift Card - survey for gift card

Tried to complete the survey to receive a $10 gift card per receipt & cashier that waited on me. Searched site and could not find any survey or information on how to access it so I could complete it. Receipt states it must me completed within 3 days of the visit. This is a real farce and The Cheesecake Factory should be ashamed. I have eaten there, bought takeout today and the food, staff, & service is great, but this survey info seems to be nothing but a come on. My survey code number is 4104-50041-14051.
