Mattress $65 rebate card never received after free-shipping offer

I am pretty sure that I didn't receive rebate card from Sears.

When I bought my new mattress from Sears they offered free shipping after $65 rebate.

Okay! I ordered and I am still waiting.

I tried to contact them about this issue.

Rebate official web page is very annoying.

Anyway, If I can't receive that rebate card I will return my mattress very soon.
