Final judgement without me knowing

I just find out last week that there was a final judgement against me for a total of $5,600. I did not receive the summons or anything like that. This judgement was entered in May.

I remember back in I went to court for this case and I was forced to stipulation agreement. I made few payments and stopped to some financial crisis on my behalf.
I would like to get rid of this judgement. I need someone's advice. Do I need to contact the creditor for clarification?
