$14.95 billing every month, will NOT refund claims cannot find me in system

This company was billing me every month $14.95 for several months. I did not catch it until today.in December and January I got bill $14.95 for Pass Port to Fun. This charge came from a company affiliated with VistaPrint.com. I cantacted this billing company at 800-566-8489 about the bills. They claimed a refund woudl occur and no further billing will take place.
Than I called the 877 number and was inquiring about the $14.95 listed today as a payment to AP9 HealthPlan. I was told I called and cancelled already today. They told me about about Passport to fun, they did not however mention why they were billing me $14.94 for the Health Plan one. I was inquiring as to this and they told me they could not see where I was to be billed for this. They had no record of my name and address, or phone number listed with the charges mentioned that took place today.
They had an address for me in Ohio where I lived nearly 8 months ago, and for my phone number the man who used to have the number. They wanted my debit card number to search their records, and because I planned to cut it up and cancel it today anyway I gave it to the man named Stephanie. *yes stephanie* Anyway he still could not find me, he told me to fax my Bank Statment to them with the item underlined. I questioned this as there is alot of personal information on my statement, I contacted the bank, my banker said "do not under any circumstances give them that, it has all your information on it"
I proceeded than to cancel the debit card, and contact AddaptiveMarketing again about this. They said there was nothing I could do about it. So I am unhappy with this company who is billing me $14.95 Reoccuring fees for something they don't even know where it is going or coming, I have no benifit for my money. Please becareful with them!
I am researching how to file a formal complaint with the state of Nebraska about them. And perhaps the Attorny General.
