Never received coupons

In November I requested two coupons and I never received them. The coupons may have been delivered to the wrong address. I spoke with customer service at the TV Converter Box Program and they gave me the reference number 34021968. Would you please send me out two coupons so I can get the converter boxes I need. Thank you.
