Equest for break down of evaluation

Erica Sudderth is the insurance agent and is in charge of my claim #463809. She refuses to send a break down evaluation, to show where she has figured up a total for the loss of my 2001 Honda Accord. Erica says she isn't sending over any other faxes, she told Evon White of Citifinancial Auto on 11/21. I had also called United Insurance on 11/23, to make Erica's supervisor aware of her behavior and he said he would send Erica an e-mail to let her know she needs to send the detailed evaluation. As of 11/26 Erica has not sent over this evaluation to finish my claim. This is the only paper work that is holding this up and I have no other form of action to get this taken care of. I had complications with Erica since the first day I had spoke with her on 10/15, the day after my accident. She told me then that she wouldn't be able to speak with me for almost a week, not till the following Thursday or Friday, 10/23 or 10/24. I was only asked where the car was, so to send an appraiser over to look and take pictures of the car. Erica called me back on 10/22, which was a day earlier than what she said she could first speak with me, and said she has figured up a total loss estimate. At this time Erica had not taken any information from me and did not know I had a lien holder, tow truck charge or a police report. She never asked for it and didn't want any of the information before her estimate was wrote up. On 11/12, she then sent a form saying I needed to send in the title and the release and keys for my vehicle that was already pickup by her company on 11/03. Erica insists on giving me a hard time when I tell her that she isn't getting the proper information to handle this and she should of taken my information before, she insists on arguing with me and tells me I didn't send over particular info but I had faxed over any information I thought she might need. I needed to put in a complaint about this lady because I have no other action I can take.
