Paid-off bill not recognized despite faxed proof and cashed payment

I have phoned several times to clear bill that was paid off. They keep saying they have not received info. Have faxed info to them at least six times since October. Mailed info twice. They continue to say they have not received payment when bank showed they cashed payment in December.
