Very rude, nasty, changing prices for same service, etc

The physicians is extremely rude and unpleasant whenever you arrived at his workplace, he do not pay attention to the issue, simply hurry during your visit, and he cost various costs for that same providers, he's getting me unemployed, and also have billed me $10.00 to complete forms, after which he billed to $40.00,30days later to complete exactly the same forms. I had been launch from disablity by 12/1. I never got a notice to come back to are of 12/1. While named and enquired concerning the notice I required, notice that states, whenever you launched me in the disablity, and was I launched in those days to visit function. Physician said he was not provided me a notice declaring I was ready to return to function when he launched the disabilty. As well as stated I had been not able to return to his exercise, whick leaves me not able to be-released to function, and not able to continue gathering disabilty. Recommended physician that they're therefore unprofessional and they should not handle spending, and covered individuals in this way. Recommended physicians office that Iwill place in a on his exercise.
