This company charged my credit card $78.41 after cancelled. They supply no address, were rude, and refused my request for refund. Did not supply ANY documentation on company to send item back

IvoryWhite offered a $1.00 trial offer which I accepted. They then charged my account an additional $2.95 Shipping. Then unauthorized, charged me $78.41 more.
No company information, address, order form, what has been charged my account, when my account would be charged again.
Only information comes from UPS label: Distribution Center, 6205 Lookout Road STE D, Boulder CO, 80301-3334.
Phone number provided goes to rude, unhelpful person unwilling to remove the charge or give me return information for product they sent.
